Position: Clinical Data Analyst - Care Coordination
Location: Bradenton, FL

Job Summary:
Responsible for clinical and other data collection and maintenance. Utilizes appropriate methodologies (charts, graphs) to disseminate information in a user friendly fashion. Assist in tracers, licensure and certificate acquisition and maintenance, and employee record keeping. Essential Duties and Responsibilities
  • Maintains clinical data base regarding, chronic diseases, and other areas as assigned.
  • Assists with preparation of monthly, quarterly, and annual reports.
  • Maintains clinical outcome data as assigned.
  • Maintains clinical process data as assigned.
  • Develops spreadsheets and databases necessary for data maintenance.
  • Develops reports as requested from Manager
  • Provides clerical support to Manager
  • Maintains files of external survey reports and internal tracers.
  • Prepares quarterly reports for grant deliverables.
  • Gathers monthly reports from charge nurses, creates summary document.
  • Prepares material for long term storage.
  • Reviews Clinical Records for key measures.
  • Assists with EMR data entry.
  • Prepares patient lists for ACO program.
  • Collaborates with Patient Care Coordinators regarding contacting and scheduling patients as assigned.
  • Has an excellent understanding of telephone usage and the operation of the telephone system
  • Answers in-coming telephone calls within three (3) rings and accurately directs and/or assists the caller
  • Forwards calls accurately to the correct person or extension and ensures that someone is answering on the extension before passing the caller
  • Requirements:
  • High School Diploma
  • Florida State drivers license
  • Advanced Skill in Microsoft Outlook, Microsoft Excel, and Microsoft Word for data entry, report generating, tables, and graphs.
  • Basic trouble shooting skills
  • 2-3 years experience in Medical Office setting.
  • Excellent communication skills
  • Experience in Data Analysis, Statistics, and Performance Improvement
  • Past experience with or knowledge of PCMH, Care Coordination, and Case Management preferred. **MCR Health Services is a drug free workplace. All job applicants selected for employment are required to submit to a pre-employment drug test.
